Likely is the operative word: the carrier assigns the class, and each carrier draws its lines differently.

What Sage asks
Plain questions, and why each one matters.
- Tobacco or nicotine, now or in the last year?
- The single biggest lever. Most carriers treat you as a non-smoker after twelve months tobacco-free; vaping and nicotine patches count. At 40, $500,000 of 20-year term is $34–$55 a month in Standard and $83–$131 with tobacco (PolicyChat, September 2026).
- Weight for height, roughly?
- Build charts differ by carrier, which is why the same person can be Preferred at one and Standard at another. If you have lost weight on a GLP-1 in the last year, most carriers add about half of it back until it has held for twelve months.
- Any of these eight conditions, and are they controlled?
- Treated cholesterol with a normal ratio is Preferred at many carriers. Controlled blood pressure is Preferred or Standard. Well-controlled type 2 diabetes is Standard or a mild rating. Control, not diagnosis, sets the class.
- How old are you?
- Age does not change the class, but it changes what a class is worth. The Standard-versus-Preferred Plus gap is about 42 to 45 percent at 40 and 43 to 43 percent at 50 for $500,000 of 20-year term, derived from the table below (PolicyChat, September 2026).

Methodology
How these estimates are made.
PolicyChat life pricing tables (same estimates Sage uses), calibrated to public term-life and final-expense quoting tools, 2026. Industry estimates from published carrier rate tables, monthly premium, not a quote. The carrier confirms the price when it underwrites.
The on-page check and Sage call the same deterministic engine (POST /api/life/estimate) with your age, tobacco status, build, and conditions. It maps those answers to a likely health class, reads the monthly range for that class from the tables dated 2026-09-18, and returns the range with the improvement paths that apply. No contact details are sent, and nothing is stored. Every figure on this page is drawn from the same tables and carries the attribution (PolicyChat, September 2026). The things people ask first.
What are the life insurance rate classes?
Most carriers use Preferred Plus (the best rates), Preferred, Standard, and Standard with a table rating, plus a separate tobacco version of each. A table rating adds a percentage to the Standard premium: Table 2 is roughly 50 percent more than Standard and each further step adds about 25 percent. Guaranteed-acceptance policies have no classes at all.
How much cheaper is Preferred Plus than Standard?
It depends on age. For $500,000 of 20-year term, Standard runs about 42 to 45 percent more than Preferred Plus at 40 (PolicyChat, September 2026): $34–$55 a month against $24–$38 (PolicyChat, September 2026). At 30 the gap is $20–$34 against $14–$24 (PolicyChat, September 2026). Tobacco roughly doubles whichever class you are in.
When do I get non-smoker rates after quitting?
At most carriers, twelve months after your last tobacco or nicotine use, including vaping, patches, and gum. Some carriers offer Preferred non-tobacco rates only after two or three years. If you quit less than a year ago, Sage can show both the tobacco price now and the non-tobacco price to expect, and most carriers will reconsider the class after the first policy year.
Can I get Preferred rates with high blood pressure or cholesterol?
Often. Treated cholesterol with a normal ratio is Preferred at many carriers, and controlled blood pressure on medication with normal readings is Preferred or Standard depending on the carrier. Uncontrolled readings, or either condition combined with obesity or diabetes, usually mean Standard or a rating.
Does taking Ozempic or another GLP-1 change my class?
Carriers see the prescription and ask what it is for. For diabetes you are underwritten as a diabetic; for weight loss you are underwritten on build, with a common rule that about half of any weight lost in the last twelve months is added back until the loss has held for a year. Disclose it, apply at the class you qualify for today, and ask for a reconsideration once your weight has been stable.
Why does the same person get different classes from different carriers?
Each carrier publishes its own build chart, blood-pressure thresholds, and cholesterol limits, and treats family history differently. The same 45-year-old can be Preferred at one carrier and Standard at another. That is why an agency appointed with several carriers matters more than any single advertised rate.
